Houston Area Continuity of Care provides care management and personal support services for elderly and/or vulnerable clients. By using our website, submitting information, or requesting services, you agree to the following Terms and Conditions.


Services


Houston Area Continuity of Care provides care management and personal support services, which may include assistance with daily needs, communication support, coordination of care, and other non-emergency services as outlined in the client’s service agreement.


We do not provide emergency services. If there is a medical emergency, call 911 immediately or seek appropriate emergency care.


Client Information


You agree to provide accurate, complete, and current information when requesting services, completing forms, or communicating with our office. You are responsible for notifying us promptly of any changes to contact information, care needs, medication instructions, emergency contacts, or other important details.


Communication


By providing your contact information or requesting services, you consent to receive communication from us by phone, email, text message, or other reasonable methods related to scheduling, care coordination, billing, service updates, and administrative matters.


If you consent to receive text messages, message and data rates may apply. You may opt out of text messages at any time by replying STOP. For assistance, reply HELP or contact our office.


Privacy and Health Information


We make reasonable efforts to protect personal and health-related information. However, electronic communications such as email or text message may not be fully secure. By communicating with us electronically, you acknowledge and accept the risks of delays, accidental disclosure, or unauthorized access.


Service Limitations


Our services are provided according to the client’s service plan, instructions, and agreement on file. We do not guarantee any specific outcome related to health, safety, companionship, or personal care.


We do not provide services outside the scope of our agreed role. If a client requires urgent medical attention, skilled nursing, or services outside our scope, it is the responsibility of the client or responsible party to contact the appropriate medical or emergency provider.


Appointments and Cancellations


Appointments and scheduled visits are based on availability and the service agreement in place. We request reasonable notice for cancellations or changes. Missed appointments, late cancellations, or repeated rescheduling may result in fees if stated in the service agreement.


Billing and Payment


Payment terms will be provided in your service agreement, invoice, or other written arrangement. You agree to pay all charges when due. Returned payments, declined transactions, or unpaid balances may result in additional fees as allowed by law and as stated in your agreement.


Website Use


You agree not to misuse our website, forms, contact methods, or content. This includes submitting false information, attempting unauthorized access, or using our website in a way that could harm our business, clients, or systems.
